We have written a new template for data collection and reviews. You will receive this via email when you first make an appt with us. We collect data so that we can measure our quality and your functionality and “wellness”.
We also hope that you will leave a review either on Google or FB so there is information on that. Here is the template:

We apologise for all this paperwork, but we are collecting data for the University of Brighton to help make our physiotherapy provision the best we can. We measure the quality of our care and the effectiveness of our treatment.
Please can you read the attached information regarding data collection and sign the consent form if you are happy toWe also ask that you complete a questionnaire on two occasions; before the first treatment session and following the final treatment session. This is to measure how well you are functioning and your “wellness” before and after your final session.
Before your first treatment
To access the questionnaire please click on the following link: bit.ly/PhysioOutcome. You will be asked to enter a four-letter access code provided by your physiotherapist; your access code for Paul is VVCL and for Nige it is
Please be assured that all data will remain confidential and will be kept anonymous.
Your physio can discuss with you if you have any questions when you come for your appointment.
After you last treatment
Access the questionnaire again here bit.ly/PhysioOutcome and follow the guidelines.
